The 5–6 hour operation vertically repositions the mid and lower face to refine facial and neck contours; the duration can extend when combined with a brow lift, laser resurfacing, or fat grafting. To lift the muscle and fascia in the middle of the face, the deep plane facelift requires the release of the retaining ligaments in the cheeks, neck, and jawline that would otherwise prevent the tissue from moving.
Once the ligaments are released, the muscle and soft connective tissue that make up the SMAS, or superficial musculoaponeurotic system, are carefully lifted upwards to their original position. This resolves the appearance of any sagging or drooping in the face.
Depending on the patient, if facial volume needs to be restored because of atrophy, fat grafting from other parts of the body or dermal fillers will be done before surgery. The deep plane lift is then performed immediately after.
As the deep plane facelift specifically addresses the middle and lower part of the face, other procedures, including brow lifts and blepharoplasty, may be performed in conjunction. Laser resurfacing to address sun damage and wrinkling may also be performed alongside the deep plane facelift.
Small, discreet incisions are made around the ears and hairline, allowing access to deeper facial structures. Once the lifting and repositioning are complete, the incisions are meticulously closed using fine sutures to minimize scarring.
